Registered Nurse or Registered Practical Nurse

Responsibilities

• Collaborate with patients, families, and interdisciplinary teams to formulate individualized care plans
• Evaluate treatment effectiveness and adjust care as needed
• Act as a patient advocate to protect rights, dignity, and autonomy
• Apply clinical judgment to respond to changes in patient status
• Manage responsive behaviors associated with dementia using de-escalation techniques
• Travel between patient homes in various settings including private residences and clinics
• Perform clinical duties and delegate tasks within scope of practice

Requirements

• Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
• Valid Standard First Aid and Level C CPR
• Valid G2/G driver's license and access to a reliable car
• Ability to lift 50lbs and assist clients with mobility
• Updated immunization records and TB test completed within 6-12 months
• Clear criminal background check and Vulnerable Sector Check completed within the last six months
• Proof of COVID vaccinations and Mask Fit

Preferred Qualifications

• Experience with wound care, IV medications, catheter care, and ostomy care
• Critical care background or experience with high-acuity patients
• Experience in dementia care and de-escalation strategies
• Previous experience as a community or visiting nurse
