Registered Nurse

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards
• Direct and provide nursing care to patients in their homes as prescribed by physicians
• Coordinate total plans of care with healthcare professionals and community resources from admission to discharge
• Complete clinical nursing assessments per federal/state program requirements and payer needs
• Develop and revise individualized plans of care/service plans in collaboration with community providers
• Ensure patient eligibility and medical necessity for services as defined by agency policy

Requirements

• Current and unrestricted RN licensure in the state of practice
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or public transportation

Preferred Qualifications

• 1+ years of RN experience
• Current CPR certification or ability to complete within 90 days of hire
• Ability to work independently
• Strong communication, writing, and organizational skills
