Registered Nurse

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by physicians and in compliance with regulations.
• Initiate, develop, implement, and revise plans of care in collaboration with healthcare professionals.
• Supervise care provided by home health aides and licensed practical/vocational nurses.
• Complete comprehensive assessments (OASIS) including accurate and timely medication reconciliation.
• Document patient visits per policy and payer requirements.
• Communicate effectively with physicians, patients, and family members to ensure quality care.
• Participate in on-call and weekend rotations as needed to meet patient needs.

Requirements

• Current and unrestricted RN licensure in the state of practice.
• Current Driver's License and vehicle insurance.
• Access to a dependable vehicle or public transportation.

Preferred Qualifications

• Current CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.
