Registered Nurse

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by physicians
• Initiate, develop, implement, and revise plans of care in collaboration with healthcare professionals
• Supervise care provided by home health aides and licensed practical/vocational nurses
• Complete comprehensive assessments (OASIS) including accurate medication reconciliation
• Document patient visits timely per policy and payer requirements
• Communicate effectively with physicians, patients, and family members to ensure quality care
• Participate in on-call and weekend rotations as needed to meet patient needs

Requirements

• Current and unrestricted RN licensure in the state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle
• Current CPR Certification
• Ability to function in any home situation regardless of client demographics

Preferred Qualifications

• 1+ years of Home Health experience
• Ability to work independently
• Solid communication, writing, and organizational skills
