Registered Nurse

Responsibilities

• Conduct comprehensive and timely delivery of hospice care for patients and families during afterhours shifts
• Triage patient needs over the phone until an in-person visit can be provided
• Respond with urgency to crisis calls from patients and families after hours
• Perform initial nursing evaluations to determine hospice eligibility within 48 hours of referral
• Execute afterhours visits including admissions, death visits, recertifications, and routine visits
• Identify and reassess physical, psychosocial, emotional, and environmental needs of patients and families
• Document patient problems, goals, interventions, and responses to hospice care

Requirements

• Current unrestricted RN licensure in the state of practice
• Current driver's license
• Valid vehicle insurance
• Access to a dependable vehicle or public transportation

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• 1+ years of clinical experience
