About the role

Responsibilities

  • Provide culturally sensitive, age-specific, team-based nursing care for primary care patients, including adult medicine, pediatrics, palliative care, and OB/GYN populations.
  • Perform phone triage, including assessment, screening, clinical advice, and nurse visits (in-person or video).
  • Execute in-clinic hands-on procedures and manage complex clinical issues using evidence-based interventions.
  • Ensure accurate and efficient documentation of nursing care within electronic health records.
  • Provide functional supervision to assistive personnel and collaborate with multidisciplinary teams to promote continuity of care.
  • Educate and coach patients and families regarding chronic conditions such as diabetes, asthma, and coronary artery disease.

Requirements

  • Current RN licensure to practice nursing in the state of Michigan.
  • Three (3) years of RN experience in Adult Internal Medicine (including Adult Inpatient, Emergency Services, or Ambulatory Care Medicine) within the last 7 years.
  • One (1) year of OB/GYN RN experience.
  • Proficiency with electronic health record (EHR) documentation and typing.
  • Strong critical thinking, problem-solving, and ability to prioritize tasks in a changing environment.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Certification in Ambulatory Nursing.
  • Experience in ambulatory phone triage.
  • Previous experience providing care to Family Medicine, OB, pediatric, or Palliative Care patients in an ambulatory setting.
