Registered Nurse

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs
• Provide professional nursing care utilizing the nursing process and palliative actions
• Develop, implement, and revise patient care plans based on nursing diagnosis and changing status
• Administer medications and treatments as prescribed by physicians
• Counsel patients and families regarding nursing needs and self-care techniques
• Act as Case Manager to coordinate patient care for assigned caseloads
• Maintain accurate clinical notes and communicate patient status changes to physicians
• Participate in on-call duties and interdisciplinary group meetings

Requirements

• Graduate of an accredited school of nursing
• Current State nursing licensure
• CPR certification
• Valid driver's license and reliable, insured automobile
• Ability to lift, position, and transfer patients up to 50 pounds
• Ability to perform physical tasks including walking, standing, stooping, and kneeling

Preferred Qualifications

• Bachelor's degree in nursing
• 1-2 years of recent acute care experience
• 1-2 years of recent home care experience
• 1 year of hospice care experience
