Responsibilities

• Complete initial and comprehensive assessments of patients and families to determine hospice needs
• Provide professional nursing care utilizing the nursing process and health assessment data
• Develop and revise care plans that establish goals and incorporate palliative nursing actions
• Administer medications and treatments as prescribed by the physician
• Assist patients with activities of daily living and facilitate comfort care
• Coordinate patient care for the agency after hours and support Case Managers
• Maintain accurate clinical notes regarding patient condition and symptom management
• Communicate with physicians regarding patient needs and obtain orders
• Teach patients and caregivers self-care techniques and medication instructions

Requirements

• Bachelor's or Associate's degree in nursing or related fields
• Active nursing license in good standing
• Active CPR certification
• Proficiency in MS Office and ability to learn job-specific software
• Strong organizational, communication, and analytical skills
• Ability to lift and/or move up to 50 pounds

Preferred Qualifications

• 2-5 years of experience in healthcare management, hospice, or palliative care
• 1 year of experience with Medicare Hospice Certification and CHAP guidelines
