Registered Nurse Liaison

Responsibilities

• Facilitate hospice referrals and act as a liaison between referral partners and the agency
• Perform physical, social, psychological, and financial assessments of patients to determine hospice eligibility
• Meet with patients and families to discuss services, manage expectations, and obtain signed consent
• Develop and maintain professional relationships with case managers, social workers, and physicians
• Initiate patient plans of care and provide education to families regarding available services
• Conduct individual and group presentations to educate the community on hospice care and philosophy
• Ensure all patient documentation is accurate and submitted to Intake Coordinators within 24 hours
• Coordinate necessary medical equipment, supplies, and transportation upon patient admission

Requirements

• Current Florida Registered Nurse (RN) license
• Minimum of two years of clinical experience in acute care settings
• Active CPR Certification
• Valid Florida driver’s license and valid automobile insurance

Preferred Qualifications

• One year of homecare and/or hospice experience
