About the role
Responsibilities
- Provide high-quality hospice care to patients in private residences, assisted living facilities, and skilled nursing homes.
- Case manage the needs of assigned patients and families, performing ongoing physical, psychological, social, and spiritual assessments.
- Create, implement, and update nursing care plans to maximize patient comfort and quality of life.
- Supervise and provide guidance to LPNs, CNAs, and other assigned field staff.
- Collaborate with the Interdisciplinary Group (IDG) and team physicians to coordinate patient care.
- Maintain accurate and timely clinical documentation using Electronic Medical Records.
- Act as a patient and family advocate while educating the community regarding hospice philosophy.
Requirements
- Current Florida RN license.
- Valid Florida driver's license and reliable transportation.
- Proof of automobile insurance.
- CPR certification.
- Proficiency in Electronic Medical Records (EMR).
- Strong critical thinking, time management, and analytical skills.
Preferred Qualifications
- Minimum of one year of nursing experience in a hospital, home health, or hospice setting.
- Experience in field-based case management.
