Registered Nurse Hospice

Responsibilities

• Provide high-quality hospice care to patients in private residences, assisted living facilities, and skilled nursing homes.
• Case manage the needs of assigned patients and families, performing ongoing physical, psychological, social, and spiritual assessments.
• Create, implement, and update nursing care plans to maximize patient comfort and quality of life.
• Supervise and provide guidance to LPNs, CNAs, and other assigned field staff.
• Collaborate with the Interdisciplinary Group (IDG) and team physicians to coordinate patient care.
• Maintain accurate and timely clinical documentation using Electronic Medical Records.
• Act as a patient and family advocate while educating the community regarding hospice philosophy.

Requirements

• Current Florida RN license.
• Valid Florida driver's license and reliable transportation.
• Proof of automobile insurance.
• CPR certification.
• Proficiency in Electronic Medical Records (EMR).
• Strong critical thinking, time management, and analytical skills.

Preferred Qualifications

• Minimum of one year of nursing experience in a hospital, home health, or hospice setting.
• Experience in field-based case management.
