Registered Nurse Homecare

Responsibilities

• Deliver skilled, compassionate nursing care directly in patients' homes.
• Complete initial and ongoing home assessments, including full physical evaluations and OASIS documentation.
• Develop individualized care plans with measurable goals, involving patients and families in the process.
• Coordinate interdisciplinary care by communicating significant changes to physicians and obtaining necessary orders.
• Teach and counsel patients and families on self-care techniques, disease management, and safety.
• Prepare timely, accurate clinical notes and maintain strong communication with the care team.
• Travel within the assigned territory to provide care to patients.

Requirements

• Associate Degree in Nursing (BSN preferred).
• Active New Hampshire Board of Nursing license.
• At least 1 year of recent acute care experience (Med/Surg or acute rehab).
• BLS/CPR certification.
• Valid Driver's license and ability to meet driving and insurance requirements.
• Familiarity with OASIS and EMR systems (Allscripts preferred).
