Registered Nurse, Home Health Assessments

Responsibilities

• Travel to members' homes across Massachusetts to conduct in-person health assessments, including Uniform Core, Functional, and Comprehensive Assessments.
• Evaluate members' physical, cognitive, and functional status with mindfulness of cultural and linguistic needs.
• Document all findings accurately in the GuidingCare care management platform within 48 hours of assessment.
• Provide SOAP notes to assigned Care Managers upon completion of visits.
• Adhere to all regulatory, privacy (HIPAA), and field-safety practices for independent in-home visits.
• Act as a mandated reporter for any suspected abuse, neglect, or exploitation of vulnerable adults.

Requirements

• Active Registered Nurse (RN) or higher licensure in good standing in the Commonwealth of Massachusetts.
• Associate of Science degree in nursing required (BSN preferred).
• Minimum of 1 year of clinical experience in a home health setting.
• Valid driver's license, personal vehicle, and verifiable insurance.
• Ability to travel to members' homes and community settings regularly (may exceed 50% of the time).
• Must reside within a commutable distance of the assigned Massachusetts territory.

Preferred Qualifications

• Minimum of 1 year of experience specifically completing health assessments.
• Prior experience working with dual-eligible populations, Medicaid, or Medicare managed care.
• Assessment-related certification, such as RAC-CT.
