Registered Nurse, Home Health Assessments

Responsibilities

• Travel to members' homes and community settings to conduct in-person health assessments.
• Perform Uniform Core Assessments (UCA), Functional Assessments (FA), and Comprehensive Assessments.
• Evaluate members' physical, cognitive, and functional status with mindfulness of cultural and linguistic needs.
• Document all findings accurately in the GuidingCare care management platform within 48 hours.
• Send SOAP notes to assigned Care Managers upon completion of assessments.
• Adhere to all regulatory, privacy (HIPAA), and field-safety practices for independent in-home visits.
• Report any suspected abuse, neglect, or exploitation of vulnerable adults as a mandated reporter.

Requirements

• Active Registered Nurse (RN) or higher licensure in good standing in Massachusetts.
• Associate of Science degree in nursing required (BSN preferred).
• Minimum of 1 year of clinical experience in a home health setting.
• Valid driver's license, personal vehicle, and verifiable insurance.
• Ability to travel to members' homes, which may exceed 50% of the time.
• Ability to work independently and manage a daily visit schedule.
• Proficiency in documenting via electronic platforms and secure virtual desktops.

Preferred Qualifications

• Minimum of 1 year of experience specifically completing health assessments.
• Prior experience with dual-eligible populations, Medicaid, or Medicare managed care.
• Assessment-related certification (e.g., RAC-CT).
