About the role
Responsibilities
- Deliver clinical nursing care including assessments, care planning, and ongoing reviews
- Perform a wide range of clinical assessments such as Falls Risk, Cognitive, Continence, Pain, Skin Integrity, and Wound care
- Provide wound care and assist with continence management and catheter care
- Collaborate with the Client Care Manager to enhance home care client outcomes
- Monitor service quality to ensure care aligns with regulatory and organisational standards
- Build strong relationships with customers, carers, and key healthcare stakeholders
Requirements
- Bachelor of Nursing and current unrestricted AHPRA registration
- Minimum 5 years of post-graduate clinical experience (ideally in aged care or community care)
- Current First Aid Certificate (HLTAID011) and Basic Life Support (HLTAID010)
- National Police Check suitable for aged care
- Full driver’s licence and access to your own vehicle
- Strong understanding of aged care standards, WHS, and risk management practices
Preferred Qualifications
- Experience in palliative care
- Experience in dementia care
