About the role
Responsibilities
- Provide compassionate, patient-centered hospice nursing care and comprehensive case management for patients in the Hospice House.
- Develop and implement interdisciplinary plans of care, revising goals every 14 days as needed.
- Collect and document accurate health histories, physical assessments, and nutritional, emotional, and spiritual needs.
- Supervise Hospice LNAs and LPNs in accordance with agency policy and state/federal regulations.
- Act as a primary advocate for patients and families, providing support through the grief and bereavement process.
- Teach patients and families self-care concepts and assist in modifying environments to promote comfort and safety.
- Participate in interdisciplinary team (IDT) meetings, patient care conferences, and staff meetings.
Requirements
- Graduate of an accredited nursing program.
- Minimum of 2 years of acute care nursing experience.
- 3 to 4 years of recent RN experience in home health or hospice, or medical/surgical in an acute or skilled long-term care setting.
- Ability to work the weekend schedule: Friday 3:00 p.m. - 11:00 p.m., and Saturday & Sunday 7:00 a.m. - 7:00 p.m.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Hospice and Palliative Care Certification.
- 2 years of oncology experience.
- Experience with IV Infusion Therapy.
