Responsibilities

• Conduct nursing triage and assessment of acute and chronic conditions
• Support medical providers during clinical visits and procedures
• Provide direct patient care including wound care, point-of-care testing, and health screenings
• Administer medications, vaccines, and PrEP per provider orders
• Maintain and monitor medication and vaccine inventory and cold chain management
• Coordinate care with interdisciplinary teams including medical, behavioral health, and residential staff
• Document patient care in the EHR (eClinicalWorks) accurately and timely
• Manage lab orders, results tracking, recalls, and follow-up
• Support preventive care outreach and chronic disease management

Requirements

• Bachelor’s degree in nursing
• Current New York State RN license
• BLS certification
• Strong clinical and technical nursing skills
• Experience with EHR systems

• Experience with underserved youth populations
