About the role
Responsibilities
- Monitor resident health and respond to changes in condition
- Complete daily rounds to support quality care
- Communicate with residents, families, physicians, pharmacists, and care team members
- Assist with pre-admission assessments and admission documentation
- Coordinate resident care plans and attend care conferences
- Support MDS compliance, clinical documentation, and routine compliance audits
- Serve as a clinical resource during emergencies or resident changes of condition
- Support frontline staff through coaching, supervision, and performance feedback
- Participate in on-call responsibilities as needed
Requirements
- Current Colorado Registered Nursing (RN) license in good standing
- 1+ year of long-term care facility experience
- 1+ year in a supervisory role
- Proficiency in Microsoft Office (Excel, Word, Outlook)
- Strong clinical judgment and communication skills
- Ability to handle objects up to 50 pounds occasionally
Preferred Qualifications
- Advanced training in geriatrics
- Experience in senior living, skilled nursing, or rehabilitation
