Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families
• Provide professional nursing care utilizing the nursing process
• Develop, initiate, and revise patient care plans based on nursing diagnosis
• Administer medications and treatments as prescribed by physicians
• Counsel patients and families regarding nursing and related needs
• Coordinate patient care and act as Case Manager for assigned caseloads
• Maintain accurate clinical notes and communicate changes in condition to physicians
• Teach patients and caregivers self-care techniques and medication instructions
• Participate in on-call duties and interdisciplinary group meetings
• Supervise ancillary personnel and delegate responsibilities as required

Requirements

• Graduate of an accredited school of nursing
• Current State nursing licensure
• CPR certification
• Valid driver's license and reliable, insured automobile
• Ability to lift, position, and transfer patients (up to 50 pounds)
• Ability to perform prolonged walking, standing, reaching, and bending

Preferred Qualifications

• Bachelor's degree
• 1-2 years of recent acute care experience
• 1-2 years of recent home care experience
• 1 year of hospice care experience
