Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs
• Provide professional nursing care utilizing the nursing process
• Develop, initiate, and revise patient care plans based on nursing diagnosis and palliative needs
• Administer medications and treatments as prescribed by physicians
• Counsel patients and families regarding nursing needs and health care instructions
• Coordinate patient care as a Case Manager for assigned caseloads
• Maintain accurate clinical notes and communicate patient status changes to physicians
• Participate in on-call duties and interdisciplinary group meetings
• Supervise ancillary personnel and hospice aides

Requirements

• Graduate of an accredited school of nursing
• Current State nursing licensure
• CPR certification
• Valid driver's license and reliable, insured automobile
• Ability to lift, position, and transfer patients (up to 50 pounds)
• Ability to perform physical tasks including walking, standing, reaching, and bending

Preferred Qualifications

• Bachelor's degree
• 1-2 years of recent acute care experience
• 1-2 years of recent home care experience
• 1 year of hospice care experience
