Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families
• Provide professional nursing care utilizing the nursing process
• Write, initiate, and revise patient plans of care based on status changes
• Administer medications and treatments as prescribed by physicians
• Counsel patients and families regarding nursing and related needs
• Coordinate care with physicians and community health-related persons
• Maintain accurate clinical notes and record symptom management outcomes
• Supervise ancillary personnel and hospice aides
• Participate in on-call duties and interdisciplinary group meetings

Requirements

• Graduate of an accredited school of nursing
• Current State nursing licensure
• CPR certification
• Valid driver's license and reliable, insured automobile
• Ability to lift, position, and transfer patients up to 50 pounds
• Ability to perform prolonged walking, standing, reaching, and bending

Preferred Qualifications

• Bachelor's degree
• One to two years of recent acute care experience
• One to two years of recent home care experience
• One year of hospice care experience
