About the role
Responsibilities
- Manage a diverse caseload ensuring quality patient care and efficient coordination
- Provide coaching and oversight to Home Health Aides to ensure clinical standards
- Develop individualized care plans and coordinate with PCPs and therapy providers
Requirements
- Associate's or Baccalaureate Degree in Nursing
- Current and valid Registered Nurse license in Massachusetts
- 2 years of clinical RN experience
Preferred Qualifications
- Home health experience
- Case management or supervisory experience
- Knowledge of OASIS or Medicaid paid family caregiving programs
