Registered Nurse Case Manager

Responsibilities

• Manage a diverse caseload, ensuring quality patient care and efficient care coordination
• Provide coaching, oversight, and support to Home Health Aides (HHAs)
• Develop and implement individualized care plans based on patient assessments and goals
• Coordinate with PCPs, therapy providers, and internal support teams
• Provide education and support to patients and families regarding health care needs
• Maintain accurate and timely electronic charting with a 24-hour turnaround
• Participate in on-call rotation, holiday rotation, and possible phone triage night coverage
• Monitor and work to improve patient readmission rates

Requirements

• Associate's Degree or Baccalaureate School of Nursing Degree
• Current and valid Registered Nurse license in the state of Indiana
• 2 years of experience as a Registered Nurse in a clinical setting
• Reliable transportation, valid driver's license, and current auto insurance
• Ability to work flexible hours and travel locally

Preferred Qualifications

• 1 year of home health experience
• Case management or supervisor experience
• Pediatric or adult care experience
• Knowledge of Medicaid paid family caregiving programs and HHA scope of practice
• Knowledge of OASIS
