About the role
Responsibilities
- Identify patients with Heart Failure (HF), Ischemic Heart Disease (IHD), and those requiring Cardiac Rehab to ensure clinical guidelines are followed
- Triage patients, administer IV therapy under provider direction, and relay test results
- Conduct Chronic Care Model visits, focusing on patient education regarding chronic disease and support systems
- Utilize motivational interviewing techniques to assist patients with self-management goal setting and action plans
- Perform remote monitoring of patients, identifying changes in vitals and discussing disease management with providers
- Enter timely and accurate data into the EMR system to ensure complete documentation of visits and calls
- Serve as a clinical resource and consultant for LVN/MA team members
- Collaborate with the cardiology team to recommend policies, procedures, and standards for high-risk chronic disease care
Requirements
- Registered Nurse (RN) license with a current license to practice in the state of employment
- 2+ years of experience in a physician's office, clinical, or hospital setting
- Clinical experience in Cardiac, medical-surgical, and/or critical care
- Proficient knowledge of chronic diseases, specifically Heart Failure (HF) and Ischemic Heart Disease (IHD)
- Experience in patient education, motivational interviewing, and self-management goal setting
- Proficient computer skills, including Microsoft Office Suite (Word, Excel, Outlook)
- Valid Driver's License and access to reliable transportation
Preferred Qualifications
- Knowledge of managed care, referral processes, claims, and ICD-9/CPT coding
- Bilingual proficiency in English and Spanish
- Excellent verbal and written communication skills
- Strong organizational and prioritization skills
