Registered Nurse

Responsibilities

• Conduct follow-up calls with patients post-discharge in Mandarin, Cantonese, and English
• Perform medication reconciliation and provide patient education on heart health and diabetes
• Collect clinical data such as blood pressure readings to support HEDIS goals
• Collaborate with pharmacists and healthcare providers to resolve medication-related issues
• Document patient interactions and medication reconciliations in the Arine software platform
• Coordinate care with hospitals, clinics, and pharmacies
• Triage high-risk members to clinical pharmacists for in-depth reviews
• Assist with obtaining necessary referrals and authorizations
• Manage a consistent volume of outbound and inbound calls

Requirements

• Bachelor’s degree in Nursing (BSN)
• Active Registered Nurse (RN) license in New York
• Bilingual proficiency in Mandarin and/or Cantonese and English
• Prior experience in case management and care coordination
• Strong knowledge of medication management and reconciliation
• Proficiency in Google Workspace and Mac environments
• Ability to work 40 hours per week (Monday–Saturday)
• Ability to pass a background check
• Must live in and be eligible to work in the United States

Preferred Qualifications

• Experience with electronic medical records (EMR) systems
• Previous work in an ambulatory care setting
• Familiarity with pharmacy operations and medication therapies
• Certified Diabetes Educator (CDE) certification
