About the role
Responsibilities
- Manage patient care through the pre-transplant stage for adult kidney patients, including health assessments, history, and appropriate interventions.
- Provide skilled observations, telephone triage, and patient/family education regarding transplant processes.
- Facilitate continuity of care and transitions between outpatient and community providers.
- Maintain accurate and efficient documentation of nursing care within electronic health records.
- Ensure regulatory compliance with HIPAA, CMS, The Joint Commission, and UNOS.
- Utilize critical thinking and problem-solving skills to manage complex clinical issues and set priorities.
Requirements
- Current licensure to practice nursing in the State of Michigan.
- Three years of nursing experience as an RN within the last 5 years.
- At least 2 years of experience caring for patients with chronic renal disease, general medicine (multisystem care), or relevant experience within the last 5 years.
- Ability to obtain Transplant Certification (CCTC/CCTN) within three years of hire.
Preferred Qualifications
- Bachelor of Science Degree in Nursing (BSN).
- Experience in an Ambulatory Care setting (2-5 years).
- Experience caring for kidney or liver transplant patients within an Ambulatory Care Unit.
- Experience using the United Network for Organ Sharing (UNOS) system.
- Previous ICU experience (1-3 years).
