About the role
Responsibilities
- Perform comprehensive assessments in the patient's home and complete skilled documentation via Homecare Homebase
- Evaluate patient and home health environments, develop care plans, and communicate with physicians and care teams
- Treat wounds and perform infusion therapy
- Administer medications and treatments as prescribed by physicians
- Serve as a patient advocate and coordinate care with referral sources and caregivers
- Provide direct supervision of nursing clinical care team members including LVNs and HHAs
- Participate in regular case conferences and in-services
Requirements
- Current Texas Registered Nurse (RN) license
- Three years of clinical nursing experience
- Current Texas driver's license
- CPR/BLS Certification
- Experience with healthcare software, preferably Homecare Homebase
- Competency to carry out skilled procedures within the scope of the Nurse Practice Act
- Strong organization, time management, and multitasking skills
- Professional appearance and effective communication skills
