About the role
About the Company
Adaptive Home Health is building a higher-acuity, patient-centered home health model. Our mission is to dramatically improve patient access to home health care through technology-driven support for field clinicians and a focus on reducing administrative friction.
Responsibilities
- Own patient assessments and skilled visit documentation in Homecare Homebase (HCHB)
- Deliver high-acuity home health care, focusing on wound care and infusion therapy
- Build and maintain plans of care through communication with physicians, care teams, and families
- Drive case management, care coordination, and barrier resolution
- Administer medications and treatments and provide patient/caregiver teaching
- Supervise and guide LVNs and HHAs when applicable
- Communicate changes in condition and coordinate services across disciplines
Requirements
- Current Texas RN license
- 3+ years of clinical nursing experience
- Proficiency with wound care and infusions
- Current CPR (BLS) certification
- Current Texas driver's license and comfort with field-based work
- Strong time management and organizational skills
- Professional communication skills
Preferred Qualifications
- Previous home health experience
- Familiarity with Homecare Homebase (HCHB) or similar EMR
- Experience coordinating care for medically complex patients
- Experience in Critical or Intensive Care
