Registered Nurse

Responsibilities

• Deliver comprehensive in-home assessments and skilled documentation including OASIS, SOC, recert, ROC, and discharge
• Provide direct, high-acuity patient care with a focus on wound care
• Build and manage plans of care with physicians
• Supervise LPNs and HHAs on assigned cases
• Act as a clinical ambassador to patients, families, and referral sources
• Drive low avoidable-hospitalization rates and strong patient-experience scores

Requirements

• Active Michigan RN license
• Reliable transportation, valid driver's license, and auto insurance
• Home health and OASIS experience strongly preferred
