About the role
Responsibilities
- Identify and monitor level of nutrition and hydration for all residents
- Complete scheduled RAIMDS assessments and assess nutritional status upon admission or change in status
- Prioritize and respond to nutrition referrals in a timely manner
- Develop, evaluate and modify nutritional care plans
- Conduct swallowing assessments as needed
- Provide nutrition counselling and education to residents and families
- Participate in Care Conferences and committee meetings
- Review and approve seasonal menus and develop individualized menus
- Provide education and training to dietary and nursing staff
Requirements
- Current registration in good standing with the College of Dietitians of Ontario
- Ability to carry own Professional Liability Insurance
- Clear Vulnerable Sector Screen
- Computer literacy with knowledge of EMR programs
- Strong leadership and organizational skills
Preferred Qualifications
- Minimum 2 years relevant experience in a long term care home
- Membership in the Dietitian Gerontology Network of Canada
- Experience with swallowing assessments
