Referral Coordinator

Responsibilities

• Send all outgoing home care and hospice referrals within specified time frames
• Verify outgoing referrals are sent to participating providers within the patient's insurance plan
• Verify 485s/Plan of care are properly signed and sent to the proper agency and billing department
• Communicate with providers, families, or facilities regarding referrals
• Track outstanding referrals to ensure timely start of care
• Update patient EMR with home care and hospice provider information
• Anticipate and respond to customer needs with sensitivity

Requirements

• High School Diploma or GED
• Minimum one year of experience in a healthcare setting
• Detail-oriented with multitasking capabilities
• Strong computer skills

Preferred Qualifications

• Experience in physician offices, home care, or hospice
