Their mission is to empower individuals and families to recover from addiction, trauma, mental illness, and other chronic medical conditions through comprehensive, integrated care.

Responsibilities

  • Implement outreach and community mapping strategies to reach high-risk Latine populations
  • Conduct assertive outreach to correctional facilities, specialty courts, and parole/probation boards
  • Perform LS/CMI assessments to identify reentry and behavioral health needs
  • Design reentry plans covering health insurance, housing, and employment
  • Provide advocacy support to clients during court appointments and hearings
  • Facilitate support groups and reentry clinics both virtually and in-person
  • Document all client encounters and data in electronic health records
  • Collaborate with multidisciplinary teams to ensure coordinated care

Requirements

  • Bilingual Spanish/English fluency required
  • Minimum associate degree in a related field
  • Experience in outreach for SUD, addiction, mental illness, or criminal justice involvement
  • Valid Massachusetts Driver’s License
  • Ability to travel to correctional facilities
  • Understanding of harm reduction techniques
  • Ability to work flexible hours including evenings and weekends

Preferred Qualifications

  • CAC certification (must be obtained within 30 days of employment)
  • Human Subjects Certification (must be obtained within 30 days of employment)
  • Familiarity with social and cultural issues specific to the Latine population
