Casa Esperanza, Inc. is a bilingual/bicultural behavioral health treatment provider serving Boston and the Merrimack Valley. Their mission is to empower individuals and families to recover from addiction, trauma, mental illness, and other chronic medical conditions through comprehensive, integrated care.

Responsibilities

  • Deliver recovery and wellness-oriented community-based case management and care coordination
  • Develop recovery or wellness plans with clients that reflect their unique strengths and needs
  • Provide emotional and social support using motivational interviewing and evidence-based practices
  • Share personal recovery experience to mentor and support clients
  • Maintain a caseload of approximately 22 individuals
  • Facilitate connections to SUD and mental health treatment, housing, and vocational services
  • Act as a recovery liaison and advocate within healthcare and social service systems
  • Conduct intensive outreach to reengage or maintain engagement with clients
  • Maintain accurate client data and encounter notes in the Evolv electronic health record

Requirements

  • Bilingual Spanish/English fluency required
  • High school diploma or GED
  • Certified Addiction Recovery Coach (CARC) certification or actively working toward it
  • Knowledge of SUD and mental health treatment and recovery resources
  • Experience with outreach and engagement for justice-involved or high-risk populations
  • Willingness to share lived experience of recovery
  • Ability to work weekends and evenings as needed
  • Understanding of harm reduction principles
  • Proficiency in Microsoft Word

Preferred Qualifications

  • Familiarity with social, health, and cultural issues specific to the Latine population
  • Experience with crisis management
