About the role
Responsibilities
- Provide person-centered, recovery-focused case management to support participants in building independence and wellbeing
- Develop, implement, and regularly review individualized, strengths-based support plans aligned with participant goals
- Deliver outreach and community-based support, including assistance with daily living skills and practical needs
- Conduct intake, assessment, and transition planning for program entry and exit
- Build respectful relationships with participants to promote choice, dignity, and self-determination
- Collaborate with internal teams and external services to coordinate holistic support
- Maintain accurate case notes, reports, and data entry in compliance with organizational and funding requirements
Requirements
- Proficiency or fluency in English and at least one additional language: Arabic, Mandarin, or Cantonese
- A related degree, diploma, or minimum Cert IV in Mental Health or Peer Work
- Demonstrated experience in case management and providing practical support to people with complex needs
- Understanding of recovery-focused, person-centered practice and psychosocial support
- Current valid Working with Children Check (WWCC) and a valid Driver's licence
- Strong interpersonal, communication, and organizational skills
