Recovery Case Worker

Responsibilities

• Provide person-centered, recovery-focused case management to support participants in building independence and wellbeing
• Develop, implement, and regularly review individualized, strengths-based support plans aligned with participant goals
• Deliver outreach and community-based support, including assistance with daily living skills and practical needs
• Conduct intake, assessment, and transition planning for program entry and exit
• Build respectful relationships with participants to promote choice, dignity, and self-determination
• Collaborate with internal teams and external services to coordinate holistic support
• Maintain accurate case notes, reports, and data entry in compliance with organizational and funding requirements

Requirements

• Proficiency or fluency in English and at least one additional language: Arabic, Mandarin, or Cantonese
• A related degree, diploma, or minimum Cert IV in Mental Health or Peer Work
• Demonstrated experience in case management and providing practical support to people with complex needs
• Understanding of recovery-focused, person-centered practice and psychosocial support
• Current valid Working with Children Check (WWCC) and a valid Driver's licence
• Strong interpersonal, communication, and organizational skills
