About the role
Responsibilities
- Provide person-centred, recovery-oriented case management to support clients in achieving individual goals and wellbeing outcomes.
- Develop, implement, and regularly review holistic, strengths-based case plans in collaboration with clients.
- Deliver culturally safe support that recognizes and responds to the needs and experiences of Aboriginal people and communities.
- Conduct client intake, assessments, and ongoing monitoring of progress.
- Support clients with daily living activities to build capacity for independent living.
- Deliver outreach services and assist clients in accessing community, health, and support services.
- Build and maintain strong relationships with internal teams, external stakeholders, and local Aboriginal communities.
- Complete accurate documentation, reporting, and administrative tasks to ensure service compliance.
Requirements
- Must identify as Aboriginal and/or Torres Strait Islander and be accepted as such in the community.
- Associate Diploma, degree, or equivalent experience in community services or a related field.
- Experience working in mental health and/or community services.
- Current valid Working with Children Check and a Driver's Licence.
- Strong understanding of recovery-oriented practice, mental health systems, and psychosocial rehabilitation.
- Well-developed communication, interpersonal, and digital literacy skills.
