Recovery Case Worker

Responsibilities

• Provide person-centred, recovery-oriented case management to support clients in achieving individual goals and wellbeing outcomes.
• Develop, implement, and regularly review holistic, strengths-based case plans in collaboration with clients.
• Deliver culturally safe support that recognizes and responds to the needs and experiences of Aboriginal people and communities.
• Conduct client intake, assessments, and ongoing monitoring of progress.
• Support clients with daily living activities to build capacity for independent living.
• Deliver outreach services and assist clients in accessing community, health, and support services.
• Build and maintain strong relationships with internal teams, external stakeholders, and local Aboriginal communities.
• Complete accurate documentation, reporting, and administrative tasks to ensure service compliance.

Requirements

• Must identify as Aboriginal and/or Torres Strait Islander and be accepted as such in the community.
• Associate Diploma, degree, or equivalent experience in community services or a related field.
• Experience working in mental health and/or community services.
• Current valid Working with Children Check and a Driver's Licence.
• Strong understanding of recovery-oriented practice, mental health systems, and psychosocial rehabilitation.
• Well-developed communication, interpersonal, and digital literacy skills.
