Requirements
Baccalaureate degree and 2 years community work experience, High school diploma and 6 years community work experience, Minimum 1 year of relevant experience
About the role
Responsibilities
- Manage relationships within detention to coordinate service and discharge plans for youth
- Establish relationships with community-based organizations to develop a provider network
- Engage youth in identifying individual strengths, needs, and interests
- Coordinate with DYFJ case management, programs, detention services, and DOE staff
- Ensure youth access services including mental health, medical, literacy, and vocational support
- Collaborate with City agencies such as DYFC, DOE, and DOC for continuity of care
- Perform field visits to coordinate medical, mental health, and educational continuity
- Participate in milieu-based meetings and program planning to reduce reoffending risks
- Prepare and maintain reports using databases, service plans, and youth surveys
- Perform Case Management duties as assigned
Requirements
- Baccalaureate degree and two years of community work experience OR
- High school graduation and six years of community work experience
- Must have at least one year of experience related to the duties described
