The organization addresses social and economic disparities through youth programming, housing, health, and education.

Responsibilities

• Conduct comprehensive initial assessments for newly enrolled families in the Non-Medical Family Home Visiting program
• Administer validated screening tools including EPDS/PHQ-9, HITS, and ASQ for depression, domestic violence, and child development
• Initiate immediate safety planning for mental health or intimate partner violence concerns and document findings for individualized service plans

Requirements

• Active, unencumbered Registered Nurse (RN) license in the State of Minnesota
• Minimum 2+ years of experience in public health nursing, maternal/child health, or home visiting
• Valid driver's license and access to reliable transportation for home visits

Preferred Qualifications

• Public health nursing experience or certification
• Experience administering standardized screening tools (EPDS, HITS, ASQ)
• Experience working with low-income families and communities of color
