Sabathani Community Center (SCC) is one of Minnesota’s oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. SCC is a major resource hub addressing social and economic disparities in youth programming, housing, health, and education.

Responsibilities

• Conduct comprehensive initial assessment visits for newly enrolled families
• Document TANF eligibility factors including income and household composition
• Assess family strengths and needs across health, housing, food security, and employment
• Administer validated screening tools including EPDS, PHQ-9, HITS, ASQ, and NCAST
• Initiate immediate safety planning for mental health or intimate partner violence concerns
• Review assessment results with families in a strengths-based, non-stigmatizing manner
• Provide findings to inform individualized family service plans
• Complete and submit assessment documentation in a timely manner

Requirements

• Active, unencumbered Registered Nurse (RN) license in the State of Minnesota
• Minimum 2+ years of experience in public health nursing, maternal/child health, or home visiting
• Valid driver's license and access to reliable transportation for home visits
• Ability to pass a criminal background check
• Ability to carry professional liability insurance

Preferred Qualifications

• Public health nursing experience or certification
• Experience administering standardized screening tools (EPDS, PHQ-9, HITS, ASQ)
• Experience working with low-income families and communities of color
