About the role
Responsibilities
- Conduct home visits to mothers residing in Department of Homeless Services (DHS) shelters who have recently given birth
- Provide health education on maternal and infant health, including breastfeeding, safe sleep, and early childhood development
- Conduct environmental assessments to ensure household safety
- Liaise with DHS staff for service coordination
- Collect and enter data, monitor, and manage case assignments
- Assist families in navigating and connecting to community resources
- Visit partner maternity facilities to identify and offer visits to eligible mothers
- Participate in program marketing and community health assessments
Requirements
- Baccalaureate degree with 12 semester credits in health education, health, social, or biological sciences; OR
- Baccalaureate degree with six months of experience in health promotion or disease intervention
- High school diploma with four years of relevant experience
- For Assignment Level II: At least one year of experience as a Public Health Adviser, Assignment Level I, or an additional year of relevant experience
