About the role
Responsibilities
- Provide consultative behavioral health to primary care providers serving patients in long-term care settings
- Manage behavioral conditions in collaboration with social workers, psychiatrists, and medical providers
- Perform comprehensive behavioral health assessments and document findings per CMS regulations
- Facilitate and implement member plans of care by engaging the multidisciplinary care team
- Coordinate care as members transition through different levels of care and settings
- Evaluate the effectiveness and cost-efficiency of care plans to improve patient outcomes and reduce unnecessary hospitalizations
- Assess the needs of members and families, making adjustments to care plans as conditions change
Requirements
- Current, unrestricted NP license in the state of Rhode Island (or ability to obtain prior to start date)
- PMHNP Certification (Psychiatric Mental Health Nurse Practitioner)
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
- 1+ years of experience in a long-term care or community setting
- Valid driver's license and access to reliable transportation for travel between care settings
Preferred Qualifications
- 3+ years of experience in a long-term care or community setting
- Demonstrated post-graduate hands-on clinical experience
- Demonstrated ability to move up to 30 pounds
