Psychiatric Mental Health Nurse Practitioner (PMHNP)

Responsibilities

• Provide consultative behavioral health to primary care providers serving patients in long-term care settings
• Manage behavioral conditions in collaboration with social workers, psychiatrists, and medical providers
• Perform comprehensive behavioral health assessments and document findings per CMS regulations
• Facilitate and implement member plans of care by engaging the multidisciplinary care team
• Coordinate care as members transition through different levels of care and settings
• Evaluate the effectiveness and cost-efficiency of care plans to improve patient outcomes and reduce unnecessary hospitalizations
• Assess the needs of members and families, making adjustments to care plans as conditions change

Requirements

• Current, unrestricted NP license in the state of Rhode Island (or ability to obtain prior to start date)
• PMHNP Certification (Psychiatric Mental Health Nurse Practitioner)
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• 1+ years of experience in a long-term care or community setting
• Valid driver's license and access to reliable transportation for travel between care settings

Preferred Qualifications

• 3+ years of experience in a long-term care or community setting
• Demonstrated post-graduate hands-on clinical experience
• Demonstrated ability to move up to 30 pounds
