Provider Growth and Relationship Lead

Responsibilities

• Actively manage and grow patient referrals in the region through systematic outreach to existing provider partners
• Identify and develop new potential referral sources to grow Amae's presence
• Coordinate rapid responses to urgent patient situations, collaborating with hospital systems, social workers, and providers
• Work closely with clinical staff to ensure referred patients are connected with appropriate care
• Serve as a visible, trusted ambassador for Amae Health in the community
• Track outreach activity, referral outcomes, and pipeline in CRM to meet or exceed enrollment targets
• Develop quarterly market plans in collaboration with clinical and leadership teams

Requirements

• 3+ years of client or customer-facing experience with defined engagement targets
• Proven track record of building a referral base in business development, sales, community outreach, or referral management
• Knowledge of and passion for mental health and wellness
• Excellent oral and written communication skills and public speaking ability
• Reliable transportation and ability to travel throughout the Bay Area
• Proficiency with Google Suite, Slack, and CRM systems
