About the role

Responsibilities

  • Perform applied research and analytic activities related to primary care delivery, population health, and health equity
  • Develop evaluation plans, research questions, and performance measures
  • Plan and coordinate practice transformation and technical assistance projects with medical practices and community organizations
  • Manage a portfolio of Patient-Centered Medical Home (PCMH) initiatives
  • Provide technical guidance and mentoring to facilitators and project staff
  • Coordinate contracts and deliverables with external vendors
  • Assist in the preparation of grant and funding application materials

Requirements

  • Master's degree in physical, biological, environmental science, or public health
  • Minimum of three years of experience in applied research, evaluation, or practice transformation in healthcare settings
  • Experience planning and evaluating projects involving healthcare providers
  • Knowledge of primary care delivery systems and healthcare operations
  • Familiarity with Electronic Health Records (EHRs) and health IT platforms
  • Understanding of population health management and Social Determinants of Health

Preferred Qualifications

  • Experience in quality improvement within public health settings
  • Strong analytic, writing, and presentation skills
  • Ability to work collaboratively in a matrixed environment
