About the role

Responsibilities

  • Oversee day-to-day coordination of HIV prevention, care coordination, and outreach services across Regions 1 and 2.
  • Supervise, mentor, and manage performance for case managers, navigators, and outreach workers.
  • Manage staff scheduling, service deployment plans, and mobile unit/community-based operations.
  • Monitor program performance against grant deliverables and implement quality improvement strategies.
  • Build and sustain partnerships with community-based organizations and service providers to expand outreach.
  • Ensure compliance with grant requirements, regulatory standards, and agency protocols.
  • Manage program inventory, including testing supplies and outreach materials.

Requirements

  • Bachelor's degree or CASAC certification.
  • At least 1 year of case management experience.
  • Proficiency in Microsoft Office Suite.
  • Experience working with electronic health records (EHR).
  • Ability to travel throughout Regions 1 and 2.
  • Flexibility to work evenings and weekends for community events and outreach.

Preferred Qualifications

  • 3 years of case management experience, including at least 1 year specifically in HIV services.
  • Experience working with underserved populations impacted by HIV, housing instability, or substance use.
  • Bilingual fluency in Spanish or another language.
  • Demonstrated commitment to harm reduction and racial equity.
