Private Duty LPN

Responsibilities

• Provide high-quality clinical services within the scope of practice in accordance with the plan of care
• Deliver direct nursing care to acutely and chronically ill patients in their homes
• Document patient visits thoroughly in the electronic medical record per regulatory requirements
• Coordinate patient care activities and report changes in condition to the healthcare team
• Participate in the care coordination process, including attending patient case conferences
• Follow all applicable infection control procedures and work under the supervision of a Registered Nurse

Requirements

• Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or reliable public transportation

Preferred Qualifications

• Current CPR certification or ability to complete certification within 90 days of hire
• Ability to work flexible hours to meet identified client needs
• Strong communication, writing, and organizational skills
• Ability to work independently and manage multiple tasks simultaneously
