Primary Care Physician

Responsibilities

• Evaluate patients' acute and chronic conditions
• Manage multiple medications, mental health, and socio-emotional needs
• Provide empathetic patient education
• Work with clinical teammates to provide comprehensive primary care
• Assist with development and implementation of organizational objectives and metrics
• Proactively manage population health using a value-based care model

Requirements

• Licensed MD/DO with board certification (e.g., ABFM, ABIM)
• 3-5 years of experience managing complex diseases (diabetes, cardiovascular, menopause, etc.)
• Experience managing overall patient care and individualized care plans
• Experience in geriatric medicine, particularly for women
• Experience with poly-pharmacy management and medication reduction
• Experience with physical and gynecological examinations in elderly populations
• Ability to obtain Minnesota MD or DO license and DEA
• Valid Minnesota Driver’s License
• Fully vaccinated for communicable diseases (Hepatitis B, MMR, influenza, COVID)

Preferred Qualifications

• Experience performing clinical procedures within scope of practice
• Value-based Care (VBC) experience or training
• Experience in a start-up or new clinic environment
• Proficiency with EMR (Athena preferred) and Microsoft Suite
• Experience in mentoring or supervising provider staff
