Primary Care Physician Assistant

Responsibilities

• Evaluate patients' acute and chronic conditions
• Manage multiple medications, mental health, and social-emotional needs
• Educate patients in a thorough, professional, and empathetic manner
• Partner with care team members in assessing, planning, and providing patient care
• Devise individualized care plans
• Counsel patients on health maintenance and disease prevention

Requirements

• Licensed Nurse Practitioner (FNP-C or AGNP) or Physician Assistant (PA-C)
• 3-5 years of experience managing complex diseases (diabetes, cardiovascular, menopause, etc.)
• Experience with geriatric medicine, particularly women
• Experience with poly-pharmacy management
• Experience with physical and gynecological examinations
• Ability to maintain DEA License and NPI number
• Valid Minnesota Driver’s License
• COVID-19 vaccination and booster required

Preferred Qualifications

• Experience performing clinical procedures (skin biopsy, joint injection, endometrial biopsy)
• Value-based Care (VBC) experience or training
• Experience in a start-up or new clinic environment
• Proficiency with EMR (Athena preferred) and Microsoft Suite
• Experience in mentoring or supervising provider staff
