Prelicensed Social Worker

Responsibilities

• Provide psychosocial services to dialysis patients and their families
• Assess psychosocial needs, strengths, barriers, and adjustment to treatment
• Collaborate with the interdisciplinary care team on care planning and patient support
• Offer counseling, education, and referrals to community and vocational resources
• Support quality improvement, documentation, and patient satisfaction initiatives
• Assist with insurance, financial, admission, and continuity-of-care needs

Requirements

• Master’s degree in Social Work
• State temporary license, Registered Social Worker Clinical Intern, Associated Social Work license, or Provisional LCSW License as required
• 0–2 years of related experience
• Ability to work onsite and travel between facilities if needed
• Strong communication, documentation, and teamwork skills

Preferred Qualifications

• Experience working with chronic illness, dialysis, or renal care populations
• Familiarity with transplant referral processes and advance care planning
• Knowledge of community resources, vocational rehabilitation, and insurance support programs
