About the role
Responsibilities
- Provide outreach, training, and technical assistance to pediatric providers in private practice and clinic sites
- Collaborate with public health and child agencies to promote developmental screening and appropriate referrals
- Maintain working partnerships with County Public Health, pediatric primary care providers, and clinics
- Identify and outreach to pediatric primary care practices serving high volumes of low-income children
- Train providers and office staff on medical home concepts and referral pathways
- Conduct regular site visits to provide technical assistance and consultation
- Assist in the design, revision, and distribution of medical home and educational materials
- Collect, consolidate, and assess program data to monitor effectiveness
- Prepare project reports, policies, and procedures for senior management
Requirements
- Bachelor's degree in a work-related discipline from an accredited institution
- Two years of progressively responsible and directly related work experience
- Ability to plan, organize, prioritize, and work independently
- Strong written and verbal communication skills
- Knowledge of public health policies and program administration
Preferred Qualifications
- Experience with child health, development, or pediatric primary care settings
