About the role
Responsibilities
- Perform assessments and consultations of physical function for patients in a primary care setting.
- Develop and implement individualized treatment plans in collaboration with patients and other healthcare professionals.
- Examine and re-evaluate treatment outcomes to improve physical functioning and alleviate pain.
- Provide education to patients, families, students, and hospital staff.
- Participate in program development, practice-based research, and quality improvement initiatives.
- Perform administrative duties to ensure the efficient operation of the Family Health Team.
Requirements
- Master’s Degree in Science (Physiotherapy) or recognized equivalent.
- Registration in good standing with the College of Physiotherapists of Ontario.
- Minimum of one year of related clinical experience.
- Experience in the assessment and treatment of neurological, cardiopulmonary, and orthopedic conditions.
- Strong computer proficiency.
- Ability to work effectively within an interprofessional team and exercise sound clinical judgment.
Preferred Qualifications
- Experience in primary care or community care settings.
- Membership in the Canadian Physiotherapy Association or Ontario Physiotherapy Association.
- Experience in health promotion, prevention, and chronic disease management.
- Experience with Telus PSS EMR.
