Physiotherapist

Responsibilities

• Assess patients' needs upon discharge
• Plan and implement appropriate physiotherapy treatment programs
• Monitor patient progress during transition from hospital to home or other care settings
• Work closely with a multidisciplinary team to ensure seamless care
• Provide individualized therapy sessions and adjust treatment plans as necessary
• Educate patients and their families about care and rehabilitation

Requirements

• Degree in Physiotherapy
• Registration with the Health and Care Professions Council (HCPC)
• Experience in discharge planning and assessment
• Strong understanding of the Discharge to Assess model
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
