Physiotherapist

Responsibilities

• Conduct physiotherapy assessments in patients' own homes
• Deliver short-term reablement interventions
• Develop safe, goal-focused rehabilitation plans
• Work with wider support teams to ensure effective handover to community services
• Maintain accurate clinical records

Requirements

• HCPC registration
• Degree in Physiotherapy
• Experience in community rehabilitation, home-based assessment, or reablement
• Strong clinical reasoning and communication skills
• Ability to work independently and within a multidisciplinary team

Preferred Qualifications

• Previous NHS experience
